Physical, virtual or hybrid events form

To request an event, please complete all sections below (entering ‘N/A’ where necessary), provide as much detail as possible, and submit this form at least 4-6 weeks prior to your suggested date for the event. 

Before completing this form, please click here to find out more about the events approval process. Please be aware that if your form is incomplete or lacking information, we will ask you to resubmit an updated form, which will delay the process.

 

Please use digits only when entering your contact number and do not use spaces. If international, use 00 and then the country code. 


Here are examples of information which can be added to the free text box below or uploaded as a document.

  • sponsor forms / logos
  • cost / budget documents
  • list of people participating in the webinar and their email addresses (Invites to the practice run/live event cannot be sent without this)
  • speaker bios and pictures
  • zoom set up requirements e.g poll questions, break out rooms etc
  • powerpoint slides
  • if ticket price should be donated to a charity
  • any multiple ticket options
